This text was originally prepared under contract with Blackwell Publishing. During preparation of this text, Blackwell Publishing was purchased by Wiley Publishing. A new contract was created with Wiley Publishing. However, the editor at Wiley did not want this project and ignored it until the project just died, despite being under contract.
